The better we understand your life-style, family environment, and
expectations, the better we'll be able to make a possible match for you with one
of our pups. If there is additional
information you'd like us to have, please provide it on the back of this
application or attach additional sheets. If,
after reviewing your application, we think your family is a possible match for
one of our pups, we'll contact you to discuss an adoption in more detail.
If geographically possible, we'd like you to visit us at Golden Acres and
we'll want to visit your home and your family as well, before either of us make
